Job description

Through advocacy, resource coordination, and evidence-based practices, you'll help clients navigate challenges, access services, and make meaningful progress on their recovery journey while fostering independence and self-sufficiency.

Pay

  • Dependent on certification status Uncertified Job Title range starts at $17.50 - Uncertified Job Title will obtain certification within the first 120 days and pay will be increased Certified Job Title range starts at $19.61 - Certified Job
  • Title pay is calculated based on years of certified experience
  • Schedule: Sunday - Thursday | 8:00am – 4:00pm OR Tuesday - Saturday | 8:00am – 4:00pm

Depending on your medical plan selection, you may also contribute pre-tax dollars to a Health Savings Account (HSA), with a company match of up to $900 for individuals and $1,800 for families. Retirement Our 401(k) plan accepts both pre-tax and Roth (post-tax) contributions and includes a company match of up to 6% of your annual salary. Job Summary This title is used for Case Managers who are working toward certification.

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Essential Functions Provides psychosocial rehabilitative services as described by the Medicaid manual Supports clients in following individualized care plans and identifying case management goals Completes documentation on services provided within Valley’s standards Meets productivity expectations as assigned by supervisor Distributes medications as necessary Transports clients to appointments as necessary

Education High School diploma or equivalent Experience None Licenses/Certificates Case Manager certification CPR certification Valley de-escalation certification Driving positions require a minimum age of 21 and a current driver’s license
