Provide direct care and psychiatric rehabilitation services to patients under the direction of an RN. Assist in developing comprehensive treatment plans and maintaining concise documentation of patient responses.
Preferred candidates hold an Associate's or Bachelor's degree in Human Services, Psychology, or Sociology. 1-3 years of related healthcare experience and strong communication skills are desired.
Patient Care. Provide direct care services to patient on an individual basis, including activities such as daily living, social and psychiatric rehabilitation as directed by the RN; communicate with patient effectively and ensure they have access to required services Treatment Planning. Participate in the development of a comprehensive treatment plan, incorporating patient’s needs and wants as shared through patient surveys and direct communication with patient Documentation. Ensure concise, pertinent and complete documentation of patient’s response to treatment PREFERRED QUALIFICATIONS: Associate's degree in human services or equivalent combination of education and work experience Bachelor's degree in psychology, Sociology or Human Services 1-3 years of healthcare experience in related field Strong communication skills EDUCATION: LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. PAY RANGE: $17.85 - $20.00 CITY: Rochester POSTAL CODE: 14621 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
