Salary Range
$35.00 – $37.00 per hour, DOE
What You’ll Do: The Impact You’ll Make
- Medical Care & Coordination
- Provide basic nursing services, including injections, medication administration, and monitoring.
- Coordinate and schedule primary care and specialty medical appointments.
- Maintain medication logs in collaboration with the Program Manager for clients receiving psychiatry services.
- Coordinate with pharmacies to ensure timely delivery of client medications.
- Offer patient teaching and supportive services to increase compliance with prescribed medication regimens.
- Collaboration & Care Integration
- Work as part of an interdisciplinary team to deliver integrated care to clients with medical, mental health, and substance use needs.
- Communicate effectively with internal and external providers to ensure continuity of care.
- Provide crisis intervention and triage clients to emergency services when necessary.
- Participate in weekly staff meetings and individual supervision sessions.
- Documentation & Compliance
- Accurately document services in real time in the electronic health record or charting system.
- Remain knowledgeable about Medi-Cal benefits and assist clients with navigating the healthcare system.
- Maintain compliance with HIPAA and agency confidentiality standards.
- Other Duties
- Perform additional tasks as assigned by the Director to support program operations.
What You’ll Bring: Your Qualifications
- Education & Experience
- Current Licensed Vocational Nurse license in good standing in the State of California required.
- One to three years of clinical experience with SMI and/or complex populations preferred.
- Skills & Abilities
- Strong ability to remain professional and effective in stressful or conflict situations, including working with uncooperative clients.
- Familiarity with Electronic Health Records (EHR) and other technology tools.
- Understanding of confidentiality standards and HIPAA compliance.
- Licenses & Requirements
- Current CPR Certification preferred.
- Proof of valid California Driver’s License, clean driving record, and insurability under Alcott’s policy required.
- Live Scan clearance required.
