Our client, based in Norfolk, is seeking an experienced Qualified Social Worker to join their Home First Team, supporting adults during inpatient stays.

You will work closely with health professionals, patients and their families, both within the hospital setting and in the community, carrying out assessments and reviews to support individuals transitioning back home or into appropriate care settings.
Key Responsibilities
Core duties include
Managing a varied social work caseload within a hospital discharge setting
Undertaking person-centred assessments with patients and their families on the ward
Developing and implementing strengths-based care and support plans that promote independence and wellbeing
Supporting safe, timely and effective hospital discharges in line with the Home First model
Completing post-discharge reviews in individuals’ homes or care settings
Working collaboratively with multi-disciplinary teams, including health and social care professionals
Supporting individuals to access appropriate services, care packages and community resources
Undertaking Mental Capacity Assessments and making best interest decisions where required
Contributing to Continuing Healthcare (CHC) processes and reviews
Managing risk and safeguarding concerns in line with statutory responsibilities
Maintaining accurate, high-quality case records and documentation
Applying relevant legislation and best practice in all aspects of the role Qualifications & Experience

Experience:
Minimum of 2+ years’ post-qualified experience (including ASYE completion)
Experience working within Adult Social Care or hospital discharge settings
Proven experience completing assessments, support planning and reviews
Experience of multi-disciplinary and partnership working
Experience supporting hospital discharges or working within a Home First/reablement model is highly desirable
Experience completing Mental Capacity Assessments and best interest decisions
