The RN Case Manager performs initial and ongoing assessments and conducts home visits to support elderly individuals and adults with disabilities. They develop and maintain comprehensive care plans to ensure the quality and effectiveness of services for members.
Candidates must be a Wisconsin Licensed Registered Nurse with an Associate degree in Nursing and a valid driver's license. At least 2 years of recent experience working with older adults or persons with disabilities in a nursing capacity is required.
