The Case Manager provides education on the Medicaid waiver process and creates Individual Support Plans based on client needs and goals. They facilitate community-based team meetings and collaborate with service providers to ensure individuals access necessary supports.
Candidates must have a Bachelor's Degree in Human Services and at least one year of experience working with individuals with intellectual and developmental disabilities. A valid driver's license, reliable transportation, and a designated home office setup are required.
