Requires a Master of Social Work from an accredited school and LSW eligibility or LCSW licensure from the State of Illinois. Preference is given to candidates with 1-2 years of clinical experience in a hospital setting.
Summary: Supports the mission of Advocate Home Health Services by delivering cost-effective, outcome-focused, and patient-centered care. Assesses patients’ psychosocial needs in the home setting, develops and implements care plans to address those needs, and collaborates with the care team to coordinate services and promote positive patient outcomes. Where You Will Work: Advocate Christ Medical Center is a prominent non-profit teaching hospital located in Oak Lawn, Illinois. Advocate Christ Medical Center serves as a major referral hospital in the Midwest for several specialties, including cardiovascular services; heart, lung and kidney transplantation; neurosciences; oncology; orthopedics; and women’s health. The hospital provides emergency care for more than 100,000 patient visits annually and has one of the busiest Level I trauma centers in Illinois, serving southern portions of Chicago and Cook County, all of Will County and regions as far south as Kankakee, Illinois, west to Morris, Illinois and east to Northwest Indiana. We’re Looking For: Licensure or Certifications LSW Eligible, LCSW preferred issued by the State of Illinois Education/Experience: Masters of Social Work services from an accredited school Years of Experience: 1-2 years of experience in a clinical, hospital setting post licensure preferred. As a Medical Social Worker, you will: Provide comprehensive social work services, including assessments, care planning, interventions, and discharge coordination, to meet patients’ psychosocial needs. Collaborates with multidisciplinary teams to ensure safe, timely care transitions and access to services such as post-acute care, mental health support, and community resources. Educates and advocates for patients and families, connects them with appropriate services, and addresses issues like end-of-life planning, abuse, and social needs. Manages care progression to optimize outcomes, reduce readmissions, and ensure appropriate levels of care. Maintain documentation, compliance with regulations, and professional standards, while supporting staff training and continuing education. Physical Requirements and Working Conditions This position requires travel, therefore, will be exposed to weather and road conditions. Operates all equipment necessary to perform the job. Exposed to a normal office environment. Exposed to patient care environment. Weekend, holidays, and evening coverage per site requirement. Position requires sitting, standing, speaking, reading/writing, and walking throughout the workday.
