Nurse Practitioner – Full-Time Position Multi-Specialty Private Physician Group

We are a multi-specialty private physician group seeking a dedicated and motivated Nurse Practitioner to join our growing medical team on a full-time basis.

Qualifications

  • Active New York State Nurse Practitioner license required
  • Family Medicine, Internal Medicine, Geriatrics, Rehabilitation, or Post-Acute Care experience preferred
  • Previous experience in skilled nursing, rehabilitation, long-term care, or assisted living is a plus
  • Strong communication, clinical judgment, and organizational skills
  • Willing to train the right candidate

Responsibilities

  • Coordinate patient care with attending physicians, families, nursing staff, and facility teams
  • Communicate regularly with physicians regarding changes in patient status and treatment plans
  • Support the medical team in providing high-quality, patient-centered care
  • Participate in care coordination, documentation, and clinical follow-up
  • Help ensure continuity of care across post-acute and long-term care settings

Requirements

An active New York State Nurse Practitioner license is required. Experience in Family Medicine, Internal Medicine, Geriatrics, or skilled nursing is preferred, though the employer is willing to train the right candidate.
