Be responsible for the coordination of supports for individuals seeking services, while providing an understanding of the Brain Injury Waiver application process and eligibility criteria.
Aide individuals with referrals, intake, and eligibility for the Brain Injury Waiver.
Develop preliminary care plans with member/guardians using person centered planning.
Work with Brain Injury providers to coordinate and negotiate required services to implement care plans. Your workload will include:
Helping applicants and assisting them in completing the Brain Injury Waiver Application.
Process, log, and document the status of Brain Injury Waiver Applications.
Participating in teleconferences for members who are placed out of State while documenting their progress in treatment.
