We're looking for a detail-oriented, experienced RN to join our Home Health team in a dedicated Start of Care role, offering top-quartile pay and the tools and support to do your best work every day. Full-time / 40 hours per week Service Area & Schedule Cleveland / Northeast Ohio | Monday - Friday, standard business hours What You'll Do Conduct comprehensive Start of Care (SOC) and Resumption of Care (ROC) assessments for new and returning home health patients Complete accurate and thorough OASIS assessments in compliance with Medicare, Medicaid, and agency standards Develop individualized, evidence-based care plans in collaboration with the referring physician and interdisciplinary team Establish rapport with patients and families from day one, setting clear expectations for the home health episode of care Identify patient goals, barriers to care, and safety concerns during the initial visit Coordinate with RN Case Managers and clinical supervisors to ensure seamless handoff and continuity of care following SOC Maintain accurate and timely documentation in the EMR within required timeframes Participate in team meetings, case conferences, and ongoing professional development What Sets VNA Apart AI-assisted documentation cuts OASIS submission time by up to 90% - spend your time with patients, not paperwork 4-star CMS agency with CHAP accreditation and a 124-year legacy in Northeast Ohio Mileage reimbursement at the IRS standard rate ($0.70/mile) Paid training & CEUs through Relias Limited weekend/holiday requirements - we respect your time off Responsive support team - scheduling, intake, clinical supervisors, triage, and after-hours coverage so you're never on an island Public Service Loan Forgiveness (PSLF) eligible employer - qualifying employees may be eligible for federal student loan forgiveness after 120 payments One of the only home health agencies in Northeast Ohio with a dedicated Behavioral Health nursing program
