- RESPONSIBILITIES: Community Support for Families Parent Navigator(s) provides direct support and service coordination to families in Region 5 CSF. CSF Parent Navigators utilize strength-based, family-driven, youth-guided, culturally responsive principles to develop individualized mentoring and engagement with families. The purpose of Community Support for Families is to ensure that services are delivered in a coordinated and therapeutic manner, and are consistent with the core values and guiding principles of the Strengthening Families and WRAP-CT principles. Parent Navigators work with the CSF worker to link families to formal and informal supports in the community.

- Responsibilities:
- Caseload &
- Care: Co-manage a shared caseload of ~12 families with CSF workers; use the WRAP process to build care plans.
- Visits &
- Contact: Maintain weekly contact and conduct joint home/community visits at family convenience (including evenings/weekends; never alone).
Meetings
Help families host monthly Family Team Meetings (FTMs) to monitor care plan progress.
Resource
Linking: Connect families to community supports, FAVOR network, support groups, and crisis/safety planning.
Advocacy &
Outreach: Advocate for families in community meetings and assist with regional outreach for local Systems of Care Collaboratives.
Compliance
Document all contacts and manage family records securely and confidentially under Wellmore guidelines.
Collaboration
- Coordinate seamlessly with DCF Social Workers when involved, and attend mandatory agency trainings.

- QualificationsEducation and
- Experience: Community Support for Families Parent Navigators should possess a college degree (AA/AS or BA/BS) in a human service field (e.g., social work, marriage and family therapy, counseling, or psychology, etc.). Persons with a degree in an unrelated field may be considered if they have at least 3 years’ experience working with families and knowledgeable about community resources and services preferred. CSF Worker must have the skills necessary to meet the cultural and linguistic needs of the families to be served by the program. Bi-lingual (Spanish-English) may be required. The successful candidates must have the ability to maintain the utmost confidentiality. Must be capable of both working independently and as part of a team. A valid Connecticut driver’s license, personal transportation and valid auto insurance is required. Candidates must be willing to use their own vehicle for transportation to/from home visits; mileage will be reimbursed. Use of company vehicle may be required when transporting clients; candidates must follow all agency policies related to their use.
- LICENSURE/CERTIFICATION/REGISTRATION: Valid driver’s license, reliable transportation and valid auto insurance is required; CPR and first-aid.
