What you'll do

As a Transitions of Care Pharmacy Technician, you'll help ensure patients receive accurate medication management throughout their hospital stay and beyond.

Your responsibilities will include: ๐Ÿ’Š Collecting and documenting comprehensive medication histories upon patient admission

๐Ÿ“‹ Performing medication reconciliation to identify discrepancies for pharmacist review

๐Ÿค Collaborating with pharmacists, physicians, nurses, and case management to support safe transitions of care

๐Ÿ“ž Contacting community pharmacies, physician offices, caregivers, and patients to verify medication information

๐Ÿ–ฅ๏ธ Accurately documenting medication histories and pharmacy interventions within the electronic medical record (EMR)

๐Ÿ“ฆ Assisting with discharge medication coordination and prescription processing

๐Ÿ” Supporting medication access by identifying insurance barriers and assisting with prior authorizations or financial assistance resources, when applicable

โค๏ธ Educating patients on the importance of medication adherence and directing medication-related questions to the pharmacist

๐Ÿ›ก๏ธ Maintaining patient confidentiality while complying with all hospital policies, state regulations, and HIPAA requirements

What we're looking for: โœ” High School Diploma or GED required

โœ” Current Illinois Pharmacy Technician License required

โœ” National Pharmacy Technician Certification (CPhT) preferred

โœ” Previous hospital pharmacy or retail pharmacy experience preferred

โœ” Medication reconciliation or Transitions of Care experience preferred

โœ” Strong attention to detail and excellent organizational skills

โœ” Outstanding communication and customer service abilities

โœ” Ability to work independently while collaborating effectively with an interdisciplinary healthcare team

Requirements

Candidates must have a high school diploma and a valid Illinois Pharmacy Technician License. National Pharmacy Technician Certification is required within 90 days of hire, and health-system pharmacy experience is preferred.
