Description ConnextCare is a Federally Qualified Health Center (FQHC) dedicated to providing high-quality, comprehensive, and compassionate healthcare services to underserved and diverse communities. Our mission is to improve health outcomes by delivering accessible, patient-centered care regardless of a patient's ability to pay.

We are seeking a dedicated and mission-driven Physician (MD/DO) to join our multidisciplinary healthcare team in our Pulaski, NY location.

The Physician will provide comprehensive primary care services to patients of all ages while supporting our commitment to health equity, preventive care, and community wellness.

The ideal candidate is passionate about serving medically underserved populations and thrives in a collaborative, team-based environment.

Provide comprehensive primary care services, including preventive, acute, and chronic disease management.

Diagnose and treat a wide variety of medical conditions.

Perform physical examinations and develop individualized treatment plans.

Order, interpret, and evaluate diagnostic tests and laboratory results.

Prescribe medications and therapies in accordance with accepted medical standards.

Collaborate with behavioral health providers, care managers, specialists, and other healthcare professionals to coordinate patient care.

Maintain accurate and timely documentation within the Electronic Health Record (EHR).

Participate in quality improvement initiatives and population health management programs.

Educate patients and families regarding disease prevention, treatment options, and healthy lifestyle choices.

Support compliance with HRSA, Joint Commission, and FQHC regulatory requirements.

Participate in provider meetings, clinical committees, and continuing medical education activities as appropriate.

Requirements

Required: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution. Current unrestricted medical license in the State of New York. Board Certified in Family Medicine, Internal Medicine, or related primary care specialty. Current DEA registration and controlled substance license (if applicable). Basic Life Support (BLS) certification.

Preferred

Previous experience in an FQHC, community health center, rural health clinic, or underserved setting. Experience working with diverse patient populations. Knowledge, Skills, and Abilities Strong commitment to serving underserved and vulnerable populations. Excellent clinical judgment and diagnostic skills. Effective communication and interpersonal skills. Ability to work collaboratively within a multidisciplinary team. Proficiency with Electronic Health Records (EHR). Understanding of culturally competent and trauma-informed care principles.
