Description ConnextCare is looking to add an experienced therapist with credentials as Licensed Master Social Worker or Licensed Mental Health Counselor to its growing Outpatient Mental Health team. This individual will work out of our Fulton and Oswego offices.
The LMSW/LMHC will collaborate with the mental health providers, medical staff, nursing staff, and management team to assure clinical quality of the highest standard.
This position works with clients of all ages with emotional and behavioral concerns, with the ability to refer severe cases to appropriate community or medical agencies.
Must have the ability to carry full patient load, including all necessary documentation, treatment planning, and scheduling.
Complete other duties and responsibilities as requested by management and supervision.
Clinical Supervision is provided.
Experience with electronic health records (EHR) and electronic medical records (EMR) strongly preferred.
