  • Employer: Rockbridge Area Community Services (RACS) ROCKBRIDGE AREA COMMUNITY SERVICES RACS is currently seeking a motivated, qualified individual to work in our coverage area of Rockbridge and Bath County, and the Cities of Lexington and Buena Vista. RACS will provide training, supervision, and education opportunities specific to the position.

  • Description: The Intensive Care Coordinator (ICC) is a vital member of the Child & Adolescent Community-Based Services team, responsible for providing intensive care coordination to children and adolescents with emotional and behavioral challenges. This role supports youth who are transitioning from residential care or are at risk of out-of-home placement by helping stabilize them within their home and community. The ICC works closely with families, service providers, and community partners to develop individualized care plans rooted in family strengths, needs, and culture. The goal is to build a strong, wraparound system of support that promotes long-term success. This position is ideal for a team-oriented professional with strong communication skills, clinical insight, and a passion for making a difference in the lives of youth and families.

  • Requirements: Must meet qualifications as a Qualified Mental Health Professional at the time of hire Bachelor's degree required Minimum two years of experience working with children/adolescents in a clinical or case management role Must obtain CANS certification within 90 days of employment Completion of High-Fidelity Wraparound training as soon as available after hire Completion of all Child Mental Health Case Management and Pre-screener certification modules within 6 months of employment Why Join RACS? Requirements
