Based at CentraCare's River Campus in St. Cloud and supported by St. Cloud Hospital, the team provides specialized care for patients with chronic kidney disease, acute kidney injury, hypertension, anemia, and end-stage renal disease requiring dialysis. The practice follows patients throughout the continuum of kidney care, creating meaningful long-term relationships while helping patients navigate complex medical conditions. The nephrology team serves a broad regional population through a combination of outpatient clinic visits, inpatient hospital rounding, and outreach dialysis locations throughout Central Minnesota. This unique model allows providers to practice in diverse care settings while maintaining continuity of care for patients in both urban and rural communities. The program offers in-center hemodialysis, peritoneal dialysis, home dialysis training, and ongoing chronic kidney disease management and education.
