About the role

Assist the RN in implementing and managing the overall plan of care for hospice patients. Provide emotional support to families and coordinate nursing care with the interdisciplinary group and CNAs.

Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.

Community Healthcare of Texas is currently recruiting for an Inpatient LVN.

Position Summary

Assist the RN in the overall plan of care for the patient.

Essential functions

Assess the patient's physical, psycho-social, and spiritual condition; contribute to the plan of care in collaboration with an RN and provide palliative nursing care.

Provide emotional support for patients and families/caregivers while instructing in end-of-life care/issues.

Assists RN in case management of total plan of care of patient/support persons by coordinating with team members, communicating with IDG, and supervising CNAs.

Document nursing care and significant interactions with patient support persons and team members.

Participate in follow-up with bereaved families with various types of contact.

Assist the RN in the admission of a patient to Hospice House, including immediate crisis management and orientation.

Assist RN in the discharge of patients and coordination of transfer, including instructing patients/caregivers in follow-up care.

Assist the RN in the management of multiple patients and their support persons simultaneously in crisis with acute symptoms, including pharmacology.

  • Position qualifications
  • Licensed Vocational Nurse licensed by the State of Texas required
  • 1 year of nursing experience is required
  • Hospice/home health experience preferred
  • Proficient skill using a computer is required; Microsoft office suite experience required

Requirements

Must be a Licensed Vocational Nurse in the State of Texas with at least one year of nursing experience. Proficiency in computer use and Microsoft Office is required, while hospice or home health experience is preferred.
