Community Healthcare of Texas is currently recruiting for a Field RN.
Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.
Essential functions
Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
Provide emotional support to patients and families while instructing them in activities of daily living.
Complete daily paperwork and charting, thoroughly and accurately, according to Policy.
Evaluate the patient for hospice appropriateness and complete admission visit and documentation as needed; communicate with the Case Manager on significant findings.

Must be a Registered Nurse in Texas with at least 2 years of nursing experience and a valid driver's license. Proficiency in computer use is required, with hospice experience strongly preferred.
