About the role

The Field RN coordinates patient admissions and manages ongoing plans of care for individuals receiving hospice services. Responsibilities include assessing physical conditions, providing emotional support, and maintaining accurate clinical documentation.

Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.

Community Healthcare of Texas is currently recruiting for a Field RN.

Position Summary

Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.

Essential functions

Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.

Provide emotional support to patients and families while instructing them in activities of daily living.

Complete daily paperwork and charting, thoroughly and accurately, according to Policy.

Evaluate the patient for hospice appropriateness and complete admission visit and documentation as needed; communicate with the Case Manager on significant findings.

  • Position qualifications
  • Registered Nurse in the State of Texas
  • Hold a valid driver's license and have a dependable means of transportation.
  • 2 years of nursing experience required
  • 2+ years of experience in hospice strongly preferred
  • Proficient skill using a computer is required; Microsoft Office Suite experience preferred.

Requirements

Candidates must be a Registered Nurse in Texas with at least 2 years of nursing experience and a valid driver's license. Proficiency in computer use is required, and experience in hospice care is strongly preferred.
