Matrix Home Care is seeking experienced and compassionate Physical Therapists (PTs) for PRN home health visits in Nokomis, Lehigh Acres, and Arcadia, FL. This position is ideal for clinicians who enjoy providing individualized care in the patient’s home and are comfortable working independently while collaborating with the interdisciplinary care team. The Physical Therapist will evaluate patients, develop treatment plans, provide skilled therapy services, educate patients and caregivers, and document care in accordance with agency, state, and federal home health requirements.
Complete physical therapy evaluations and follow-up visits in the patient’s home Assess mobility, strength, balance, gait, safety, functional limitations, and home environment needs Develop individualized treatment plans and goals based on patient condition and physician orders Provide skilled therapy interventions to improve mobility, endurance, strength, coordination, safety, and independence Educate patients and caregivers on home exercise programs, transfer techniques, fall prevention, adaptive equipment, and safety precautions Monitor patient progress and update treatment plans as appropriate Communicate changes in condition, progress, and recommendations to the care team and physician as needed Complete accurate and timely documentation in accordance with home health standards and regulatory requirements Maintain compliance with agency policies, AHCA requirements, and applicable Florida home health regulations
Active Florida Physical Therapist license Prior home health experience preferred Current CPR/BLS certification preferred Valid driver’s license, reliable transportation, and auto insurance Strong assessment, documentation, time management, and communication skills Ability to work independently in the home setting Comfortable traveling within assigned service areas
