The RN will perform Start of Care assessments, skilled nursing visits, and comprehensive patient evaluations in home settings. Responsibilities include developing individualized care plans, monitoring patient conditions, and coordinating care with physicians and families.

Active Registered Nurse license in Florida or an active compact nursing license Current CPR certification Previous home health experience preferred Experience completing Start of Care visits and OASIS documentation strongly preferred Strong clinical assessment and documentation skills Ability to work independently in the field Reliable transportation and willingness to travel throughout Titusville and surrounding areas Ability to communicate professionally with patients, families, physicians, and clinical staff Current driver’s license and auto insurance
