Registered Nurse (RN) – Start of Care and Home Health Visits | East Orlando, Orange County, FL Location: East Orlando and surrounding areas in Orange County Position Type: Per Diem / Field-Based Home Health Schedule: Flexible, based on patient needs and nurse availability About Matrix Home Care Matrix Home Care is seeking an experienced and dependable Registered Nurse to complete Start of Care assessments and skilled nursing visits throughout East Orlando and the surrounding Orange County area. This position is ideal for an RN who is comfortable working independently in the field, completing comprehensive patient assessments, coordinating care, and providing high-quality nursing services in the home setting.
Complete Start of Care assessments and initial patient evaluations Perform skilled nursing visits, reassessments, recertifications, resumptions of care, and discharge visits Complete comprehensive nursing assessments and identify changes in patient condition Develop, review, and update individualized plans of care Provide wound care, medication management, disease-management education, and other skilled nursing services Educate patients and caregivers regarding medications, treatments, safety, and care instructions Coordinate with physicians, therapists, caregivers, and the clinical team Complete accurate and timely OASIS and electronic medical record documentation Report significant findings and changes in condition promptly Follow agency policies, professional nursing standards, and regulatory requirements
Active Florida Registered Nurse license or active compact nursing license Current CPR certification Previous home health experience preferred Start of Care and OASIS experience strongly preferred Strong assessment, communication, and documentation skills Ability to manage visits independently and meet documentation deadlines Reliable transportation Valid driver’s license and current auto insurance Willingness to travel throughout East Orlando and surrounding Orange County communities
