Physical therapists and assistants will conduct home health visits to perform evaluations, develop individualized treatment plans, and provide skilled therapy services. They are responsible for monitoring patient progress, educating families, and maintaining accurate clinical documentation.
Matrix Home Care is seeking experienced and dependable Physical Therapists and Physical Therapist Assistants for PRN home health visits throughout Vero Beach and Port St. Lucie. Physical Therapists will complete evaluations, reassessments, develop individualized plans of care, and provide skilled therapy services. Physical Therapist Assistants will provide follow-up treatments under the direction and supervision of the Physical Therapist. Physical Therapist Responsibilities Complete evaluations, reassessments, and discharge assessments Develop individualized plans of care and measurable treatment goals Evaluate strength, balance, gait, mobility, endurance, and fall risk Provide therapeutic exercise, gait training, transfer training, and balance interventions Recommend durable medical equipment and home-safety modifications Educate patients, caregivers, and family members Supervise Physical Therapist Assistants as required Complete accurate and timely clinical documentation Communicate changes in condition to physicians and the interdisciplinary care team Physical Therapist Assistant Responsibilities Provide treatment visits according to the Physical Therapist’s plan of care Assist patients with strengthening, mobility, transfers, gait, balance, and therapeutic exercises Reinforce home exercise programs and fall-prevention strategies Monitor and document patient progress Educate patients and caregivers on safe mobility techniques Report changes in condition to the supervising Physical Therapist Complete visit documentation accurately and on time
Physical Therapist: Current Florida Physical Therapist license Graduate of an accredited Physical Therapy program Home health experience preferred Experience completing evaluations and plans of care Current CPR certification Physical Therapist Assistant: Current Florida Physical Therapist Assistant license Graduate of an accredited Physical Therapist Assistant program Home health experience preferred Current CPR certification All applicants must have: Reliable transportation Strong clinical documentation and communication skills Ability to work independently in patients’ homes Professional liability insurance, if required Ability to successfully complete all required pre-employment screenings Background Screening Requirement Under Florida law, this role is subject to background screening pursuant to Chapter 435, Florida Statutes, including fingerprinting and a review of eligibility before employmen
