Experienced in Tracheostomy and Ventilator. Skilled in G-Tube. Assess, plan, implement, and evaluate individualized patient care plans Administer medications, treatments, and procedures in accordance with physician orders Monitor and document patient vital signs, symptoms, and progress Collaborate with physicians, specialists, and interdisciplinary care teams Educate patients and families on health conditions, treatment plans, and discharge instructions Maintain accurate and timely clinical documentation and medical records Respond promptly to patient needs and changes in condition
