Provide direct patient care including monitoring vital signs, administering medications, and wound care Skilled in Trach and vent. Assist in developing and implementing individualized patient care plans Collaborate with RNs, physicians, and healthcare team members to ensure optimal patient outcomes Document patient assessments, treatments, and progress accurately in medical records Educate patients and families on health management, medications, and discharge instructions Respond promptly to patient needs and changes in condition, escalating concerns as appropriate Maintain a safe, clean, and compliant care environment in accordance with healthcare regulations
