The Care Manager plans and delivers end-of-life care to patients in homes and facilities using the nursing process of assessment and implementation. They collaborate with families and interdisciplinary team members to maintain professional nursing standards.
This is a DRIVING position and will require travel throughout Butler/Warren/Northern Hamilton Counties, seeing patients in both homes and facilities
This is a Full-Time day shift position with an every 6th weekend rotation and 1 holiday per year

We provide superior care for patients in families in homes/facilities. The Care Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.

Candidates must hold an active Ohio Registered Nurse license, be CPR certified, and possess a valid driver's license with insurance. Two years of acute care experience and CHPN certification are preferred.
