About the role

Conduct thorough nursing assessments and manage patient pain and symptoms using evidence-based hospice practices. Coordinate care plans and provide essential education and support to both patients and their family caregivers.

A Typical Day for a Registered Nurse Case Manager with WesleyLife At Home Hospice:

  • As an RN Case Manager with WesleyLife at Home Hospice, each day begins with purpose—the opportunity to bring comfort, dignity, and compassionate support to patients and families navigating one of life’s most meaningful journeys. During your visits as a Registered Nurse Case Manager you will:
  • Travel to each patient to conduct thorough nursing assessments to monitor symptoms, comfort levels, and overall well‑being
  • Manage pain and other symptoms using evidence‑based hospice practices
  • Coordinate medications, treatments, and supplies to support patient needs
  • Prepare and update individualized care plans in collaboration with the interdisciplinary team
  • Provide education on what to expect throughout the hospice journey
  • Teach family caregivers how to provide safe, comfort‑focused care
  • Offer a listening ear and calm presence during moments of uncertainty

Open Shifts Available

• Full-Time Monday-Friday, Back-up on-call Weekday and weekends

Qualifications

  • Valid, unrestricted Iowa driver’s license.
  • Current vehicle insurance.
  • Current license as a registered nurse in the state of Iowa.
  • Two years of full-time nursing experience required, with one year of hospice experience preferred.
