Overview Works independently within the practice and serves as a practice liaison in interactions with other RMG departments to include the remote health care coach team, the Complex Case Managers, and other health care and community services. Focuses on preventive health care as it relates to annual wellness visits and closing care gaps.
What you will do
Utilizes Practice Dashboard to determine all Medicare eligible patients for Annual Wellness Visits and subsequent AWVs. Assures timely screenings of patients to allow early interventions on positive findings. Provides accurate billing detail on all services performed at the practice to the biller/coder.
Provides effective education to patients that promotes health and wellness. Provides education and information to the practice team that is timely and relevant to the goals of the medical home.
Performs pre-visits and patient outreach to assure closure in care gaps. Works quality dashboards to assure care gaps are being addressed. Care documentation is accurate, assuring patient safety. Care concerns from annual wellness visits are addressed timely to provider and/ or office manager. Completes charts with accuracy same day of service.
Demonstrates the ability to build trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings.

Candidates must be a graduate of a Licensed Practical Nurse program with at least one year of nursing experience. Required certifications include a Virginia LPN license and CPR/BLS certification from AHA, ARC, or ASHI.
