About the role

Provide personal hygiene care and companionship to hospice patients according to their individual Plan of Care. Observe and report changes in patient conditions to the Nurse or Care Manager.

Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.

Community Healthcare of Texas is currently recruiting for a Certified Nurse Aide (CNA).

Position Summary

Provides personal hygiene care to hospice patients following the patient's Plan of Care.

Essential functions

Provides personal hygiene care to patients according to the individual Plan of Care, such as bathing and shampooing.

Deliver routine medical supplies as needed to patients.

Observe and report any changes in the patient's condition to the Nurse/Care Manager.

Demonstrate respect and understanding of the philosophy, mission, and service standards of CHOT, and provide excellent customer service.

Successfully complete 12 hours of in-service training annually.

May assist the patient with transfers as needed.

Provide compassionate companionship to patients while delivering services.

Depending on the census, assignments may include patients in the field or in inpatient units.

  • Position qualifications
  • HS Degree or GED required
  • Proof of State Certification - Certified Nurse Aide required
  • 1 year of direct patient care experience required
  • Must demonstrate skills and written competency before working independently with the patient

Requirements

Requires a high school diploma or GED and a valid State Certification as a Certified Nurse Aide. Candidates must have at least one year of direct patient care experience.
