This is an excellent opportunity for a physician who enjoys practicing medicine in a value-based, managed care environment with smaller patient panels, longer appointment times, and robust clinical support. Provide comprehensive primary care services to Medicare Advantage and senior patients age 65+ Perform thorough assessments and develop individualized care plans Manage chronic conditions, preventive care, and acute medical concerns Deliver patient-centered care within a managed care/value-based care model Collaborate with physicians, specialists, and interdisciplinary care teams to coordinate treatment plans Identify and close care gaps while supporting quality and outcome initiatives Educate patients and caregivers regarding disease prevention, healthy lifestyle choices, and long-term wellness Build strong relationships with patients and families through compassionate, personalized care Maintain accurate and timely documentation within the EMR Schedule Monday – Friday 8:00 AM – 5:00 PM
MD or DO degree from an accredited medical school Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics Experience in Managed Care, Value-Based Care, or Medicare Advantage preferred New Graduates Welcome Florida Medical License preferred or eligible ACN licensed accepted Geriatric care experience preferred Strong communication and patient engagement skills Spanish-speaking ability preferred for one opening Ideal Candidate The ideal physician is passionate about caring for seniors, values quality over volume, and enjoys practicing in a collaborative environment that prioritizes patient outcomes, preventive care, and meaningful physician-patient relationships.
