The Physical Therapist is responsible for planning, implementing, and evaluating physical therapy programs within a skilled nursing facility. This includes collaborating on patient care plans, maintaining detailed treatment records, and supervising Physical Therapist Assistants.

Requirements

Candidates must hold a doctorate degree in physical therapy (or a grandfathered bachelor's/master's degree) from an accredited institution. A current valid license to practice as a physical therapist in the State of Wisconsin is required.
