Job Description

UW Medical Center - Montlake Transplant has an outstanding opportunity for a Nurse Practitioner or Physician Assistant (ARNP/PA) to join our KIDNEY Transplant Team.

WORK SCHEDULE 40 hours per week Day Shift Monday-Friday

POSITION HIGHLIGHTS The Advanced Practice Provider will provide specialized care for transplant patients and family assessment for Kidney care. This is a great opportunity to join our interdisciplinary collaborative team of outstanding providers.

PRIMARY JOB RESPONSIBILITIES Conduct initial consults, as well as follow-up visits for patients.

With keen attention to the specific needs of patients, deliver excellent care, with compassion, motivation, and sound medical knowledge.

Coordinate and monitor patients' status in conjunction with attending physicians, social workers, and other providers.

Respond to patients' healthcare needs, providing triage, urgent care and interventions as necessary/appropriate.

Follow patients, including longitudinal care and follow-up, in conjunction with other providers.

Requirements

Nationally certified ARNP/PA-C with DEA. Current and valid Washington State Registered Nurse Licensure and ARNP Licensure or Physician Assistant License. Graduate Degree from an accredited Nurse Practitioner Program or graduate of an accredited PA program. Proven excellence working in an interdisciplinary collaborative team environment. Effective interpersonal skills, leadership skills, and clinical expertise in patient care. Experience in the provision of transplant services including initial assessment, diagnostic evaluation, and management of patients.
