Position Summary

Matrix Home Care is seeking experienced and compassionate Occupational Therapists (OTs) for home health opportunities in Pasco and Pinellas Counties. This position is ideal for therapists who are dependable, patient-focused, and comfortable providing therapy services in the home setting. The Occupational Therapist will evaluate patients, develop individualized treatment plans, provide skilled therapy services, and help patients improve independence with daily activities, safety, mobility, strength, coordination, and overall function in the home. Location Pasco and Pinellas Counties Field-based / Home Health setting Schedule Flexible scheduling Per diem / PRN opportunities available

Responsibilities

Complete occupational therapy evaluations and reassessments in the home setting. Develop and implement individualized treatment plans based on patient needs and physician orders. Provide skilled therapy services to improve activities of daily living, upper body strength, coordination, safety, endurance, and functional independence. Educate patients and caregivers on home safety, adaptive equipment, energy conservation, transfer techniques, and daily living skills. Monitor patient progress and update the plan of care as needed. Communicate with physicians, clinical leadership, and the care team regarding patient status and changes in condition. Complete accurate and timely documentation. Follow company policies, home health standards, and all applicable state and federal regulations.

Requirements

Active Florida Occupational Therapist (OT) license in good standing. Previous home health experience preferred. Experience with evaluations, treatment planning, patient education, activities of daily living training, adaptive equipment, transfer training, and fall prevention preferred. Must be reliable, professional, compassionate, and patient-focused. Must be comfortable working independently in the field. Strong communication and documentation skills required. Must be able to pass all required background screening and onboarding requirements.
