Provide intensive, structured family therapy and behavioral health assessments for children and adolescents in home and community settings. Collaborate with multidisciplinary teams and external providers to ensure holistic, integrated care and maintain compliant clinical documentation.
Direct Clinical Care Provide intensive, structured family therapy in home and community settings. Conduct comprehensive behavioral health assessments and develop individualized treatment plans. Deliver ongoing therapeutic interventions using models such as CBT, TF-CBT, DBT, ARC, motivational interviewing, and family systems approaches. Team-Based & Coordinated Care Collaborate closely with a Therapeutic Training & Support (TT&S) worker as part of the IHT team model. Coordinate care with schools, primary care providers, DCF, DMH, and other service providers. Participate in wraparound meetings to ensure holistic, integrated care. Documentation & Compliance Maintain accurate, timely clinical documentation in compliance with CBHI and MassHealth standards. Utilize Electronic Health Record (EHR) systems to document services and track progress. Professional Development & Collaboration Participate in regular supervision, team meetings, and trainings. Stay current with evidence-based practices and emerging clinical approaches.
