Assess and monitor the clinical status of elders while delivering evidence-based care within a Clinical Support Team. Responsibilities include administering medications, conducting physical assessments, and mentoring care partners in a self-managed environment.
This is a day shift 6am-2pm, rotating days schedule with rotating days off.
Nurses are core members of the Clinical Support Team (CST). Their role on the CST includes assessing and monitoring the clinical status of the elders and delivering evidence-based clinical care, both directly and through the Care Partners and other CST members. The CST nurses report directly to the Director of Nursing but work in collaboration with the Guide and Care Partners.
Environment: Work is performed in a Green House home setting, where the focus is on resident-centered care and a self-managed team model. The facility is designed to promote a comfortable, home-like atmosphere, supporting both staff and residents in a collaborative, nurturing environment. Exposure to bodily fluids, infectious diseases, and other health hazards, with the consistent use of personal protective equipment (PPE) required. Compliance with infection control protocols and health and safety standards is essential.
Physical
Demands: Regular physical activity is required, including moving between residents’ rooms, performing hands-on care, and assisting with physical tasks such as lifting, transferring, and repositioning residents. Ability to lift, move, or support residents and equipment, with the capacity to lift and/or move up to 50 pounds with appropriate assistance. Must be able to stand, walk, bend, squat, and engage in other physical tasks for extended periods.
Emotional
Demands: The role requires emotional resilience and the ability to manage challenging or stressful situations, especially when dealing with resident needs, family concerns, and medical emergencies. Must maintain a compassionate and empathetic approach, providing care and support to residents and their families during emotionally sensitive moments. Ability to manage the emotional demands of working in a healthcare environment with a focus on person-centered care.
Hours and
Shift: Nurses may work various shifts, including evenings, weekends, and holidays, to ensure continuous care and support for Elders. Flexibility in scheduling is essential to meet the needs of the Green House home.
The position involves working closely with a multidisciplinary team and the home’s self-managed work team. Regularly partnering with Nurses, Care Partners, Guides, physicians, therapists, and other healthcare professionals, to provide comprehensive care. The role requires collaboration, communication, and shared decision-making with the team, promoting a culture of teamwork and support. The work environment is self-managed, meaning the team collectively handles day-to-day operations, resident needs, and staffing schedules. A supportive atmosphere is encouraged, where every team member contributes to the care of residents and the overall success of the home.
