Licensed Practical Nurse (LPN) – Home Care | Millersburg, OH

Position Details

  • Full-time, Monday-Friday, 8:00 AM - 4:30 PM
  • Based with the Millersburg team, serving the southern portion of the region
  • Extensive driving is required throughout the workday
  • Participation in a weekend rotation (currently every 9th weekend, subject to change)
  • Two holiday shifts per year
  • Mileage reimbursement provided

Essential responsibilities

Provide skilled nursing care under the direction and supervision of a Registered Nurse (RN) in accordance with the patient's plan of care and within the LPN scope of practice.

Assist with nursing assessments by observing, collecting, and reporting patient information to the RN.

Deliver care that addresses the physical, emotional, spiritual, and psychosocial needs of patients and their families.

Monitor patient condition, recognize changes in status, and promptly communicate findings to the RN.

Administer medications as prescribed using the correct dose, route, and frequency.

Educate patients and caregivers on self-care techniques as directed by the RN.

Document patient care, observations, and interventions accurately and promptly in the electronic health record.

Collaborate with the interdisciplinary team to promote patient comfort, safety, and quality outcomes.

Assist other departments and team members as needed to ensure comprehensive patient care.

Participate in Continuous Quality Improvement initiatives.

Serve as a preceptor for new team members when assigned.

Qualifications

  • Current Ohio Licensed Practical Nurse (LPN) license in good standing
  • One year of nursing experience preferred
  • Current CPR certification
  • Basic computer skills for electronic documentation
  • Valid driver's license with a safe driving record
  • State minimum automobile insurance coverage
  • Ability to travel extensively throughout the service area, including daytime, evening, and inclement weather driving

Requirements

Must hold a current Ohio LPN license, CPR certification, and a valid driver's license with insurance. One year of nursing experience is preferred along with basic computer skills for documentation.
