Essential responsibilities
Provide skilled nursing care under the direction and supervision of a Registered Nurse (RN) in accordance with the patient's plan of care and within the LPN scope of practice.
Assist with nursing assessments by observing, collecting, and reporting patient information to the RN.
Deliver care that addresses the physical, emotional, spiritual, and psychosocial needs of patients and their families.
Monitor patient condition, recognize changes in status, and promptly communicate findings to the RN.
Administer medications as prescribed using the correct dose, route, and frequency.
Educate patients and caregivers on self-care techniques as directed by the RN.
Document patient care, observations, and interventions accurately and promptly in the electronic health record.

Participate in Continuous Quality Improvement initiatives.
