We are seeking a compassionate and skilled RN Admission Care Liaison to join our South Region team. The RN Admission Care Liaison plays a vital role in ensuring patients receive timely access to hospice services. In this position, you will conduct hospice admissions, coordinate care, educate patients and families, and connect them with the resources needed to navigate end-of-life care with dignity and support. Success in this role requires strong clinical judgment, excellent communication skills, compassionate patient advocacy, and a genuine commitment to hospice and palliative care.
Key responsibilities
Complete hospice admissions efficiently and accurately.
Assess patient needs and coordinate appropriate hospice services.
Educate patients and families about hospice care and available resources.
Develop and maintain positive relationships with patients, families, referral sources, and healthcare partners.
Accurately document all patient interactions and services within the electronic medical record at the point of care.
Meet organizational performance expectations, including maintaining a 75% admission conversion rate.
Ensure compliance with all hospice regulations, policies, and standards of care.
CHPN certification is preferred and required within two years of employment. Eligibility begins after completing 500 hours of hospice or palliative care experience within the previous 12 months.
