Responsibilities Record a patient’s medical history accurately Take and record measurements of blood pressure, temperature, heart rate etc. Observe patients under treatment to identify progress, side-effects of medications etc. Monitor patients’ condition including fluid intake and output and compose patient charts Assist ailing patients in daily necessary activities such as eating, bathing etc. Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks Collect samples for lab testing as assigned Provide emotional and psychological support when needed Communicate with patients’ family or friends to provide advice, comfort and release instructions While we are looking for individuals who are dedicated healthcare professionals, we also realize that you need – and deserve – quality benefits for yourself and your family. Current, unrestricted LPN/LVN license in the state of practice. Knowledge of long-term care regulations, resident rights, and skilled nursing facility standards. Experience providing direct patient care to geriatric and long-term care populations. Ability to administer medications, treatments, and physician-ordered therapies safely and accurately. Skilled in monitoring residents' conditions and reporting changes to the interdisciplinary care team. Experience with electronic health records (EHRs) and clinical documentation. Knowledge of infection prevention and control practices. Strong assessment, communication, and critical-thinking skills. Ability to supervise and delegate tasks to Certified Nursing Assistants (CNAs) in accordance with state regulations. Ability to work effectively with residents, families, physicians, and interdisciplinary team members. Compassionate, resident-focused approach to care with a commitment to preserving dignity and quality of life. Current CPR/BLS certification.

Requirements

Candidates must be graduates of an accredited Practical Nursing program with a current, unrestricted LPN/LVN license and CPR/BLS certification. Experience in long-term care and proficiency with electronic health records are required.
