The Case Manager I provides on-site shelter services by identifying client needs and coordinating psychosocial, habilitative, and medical services. Key duties include conducting interviews, developing service plans, assisting with housing placement, and maintaining accurate case notes in HMIS.

The Case Manager I provides on-site shelter case management services to eligible clients in which an individual's needs are identified and the psychosocial, habilitative and medical services designed to meet those needs are located, coordinated and monitored in collaboration with other professional staff.

About the Role

Conducts casework interviews with clients to obtain information for formulating program/service eligibility and case status.

Identifies psychosocial, economic, and physical needs of clients; assesses and reassesses client's support systems, available community resources and other factors to plan, develop, and implement an appropriate service plan.

Develops and implements a case plan that presents intervention strategies to meet client.

Assist client with housing search and placement activities.

Refers clients to appropriate community resources and advocates for or assists the client in obtaining such services.

Prepares complete and accurate case notes; writes correspondence, reports and other written materials; inputs data into HMIS.

Reviews all assigned cases on a scheduled basis to determine continuing need for services and participate in case consultations.

Education

Bachelor’s in a social work related field preferred.

Experience

  • Two years case work experience in a social service agency, or fieldwork; SOAR experience/training preferred
  • Or any equivalent combination of education and experience that provides the requisite knowledge, skills and abilities.

Full-time employees scheduled to work 30 or more hours per week will be eligible for full benefits, including health, dental, vision and hearing benefits, basic and voluntary life benefits, holiday pay and paid time off, which is pro-rated based on their regularly scheduled work week.

Non-Exempt Employees

Health, dental, vision, and hearing benefits, as well as basic and voluntary life benefits, will begin on the first day of the month following 90 days of employment. If the completion of a 90-day waiting period occurs on the first day of the calendar month, coverage will begin on that day.

Compensation

$14.66-$18.33/hour, based on experience.

Requirements

A Bachelor's degree in a social work related field is preferred, along with two years of casework experience in a social service agency. SOAR experience or training is also preferred.
