About the role

The LPN provides direct patient care, monitors resident health, and administers medications in a skilled nursing and assisted living environment. They act as a critical link between residents, families, and physicians while documenting medical records and executing physician orders.

A Licensed Practical Nurse plays a vital role in our Skilled Nursing and Assisted Living facility by providing direct patient care, monitoring residents’ health, administering medications, and communicating changes in condition to the healthcare team. They help ensure residents receive safe, consistent, and compassionate care while supporting daily quality of life. LPNs also serve as an important link between residents, families, caregivers, and supervising nurses or physicians.

In This Role You Will

Maintains responsibility for admission and assessment of new residents.

Makes routine rounds of assigned residents and modifies standard nursing procedures for care, as needed.

Ensures that medications are administered within required time frames and as needed (PRN). Documents any adverse reactions and takes appropriate measures.

Inventories medication supplies and orders medications from the pharmacy as needed. Reads all medication labels and alerts supervisor of any possible pharmacy or dispensing errors.

Assists physicians in making rounds, performs nursing procedures, and contributes to residents’ care plan and progress.

Observes and reports symptoms/conditions of residents to the treating physician, director of nursing, and designated family member(s) in a timely manner.

Receives, transcribes, and executes physicians’ written or verbal orders.

Documents pertinent information in residents’ medical records according to departmental policy and applicable State Department of Health regulations.

Monitors and assists with responding to all nurse call systems and reports malfunctions immediately to the nursing leadership.

Uses personal protective equipment when providing care and ensures all medical and medication dispensing equipment is clean, sanitized, secured and kept in an orderly manner.

Shift Details

  • Fulltime
  • Dayshift position: 7a-730p, every other weekend and some holidays
  • PRN position: staff are required to do 3 shifts per month to maintain status

Who You Are

  • Graduate of an accredited school of Nursing program
  • Maintain an active license in good standing in the State of Ohio
  • A compassionate, team player, and have passion to work with the elderly!

Requirements

Candidates must be a graduate of an accredited nursing program and hold an active license in good standing in the State of Ohio. A compassionate team player with a passion for working with the elderly is required.
