The Licensed Practical Nurse- Home Health
Provide safe, high-quality home health care.
Follow nursing best practices and agency policies.
Communicate effectively with clients, families, and the care team.

This part-time LPN role offers a flexible schedule with a few shifts per week. The LPN will provide nursing care to patients, support their health needs, communicate with families and the care team, and complete required documentation. The position also includes participation in an on-call rotation for occasional clinical needs.
Graduate of an approved school of practical nursing, currently licensed as a Licensed Practical Nurse in the State of Indiana
Two (2) years of experience in home health care or related health field (Preferred)
Car with required proof of insurance coverage and an Indiana Driver’s license
Knowledge of all types of home health equipment utilized by patients as well as forms and paperwork necessary for documentation
