In This Role You Will

The Licensed Practical Nurse- Home Health

Provide safe, high-quality home health care.

Follow nursing best practices and agency policies.

Communicate effectively with clients, families, and the care team.

Educate clients, families, and staff as needed.

Use equipment and supplies efficiently.

Complete documentation accurately and on time.

Support professional growth and teamwork.

Shift Details

This part-time LPN role offers a flexible schedule with a few shifts per week. The LPN will provide nursing care to patients, support their health needs, communicate with families and the care team, and complete required documentation. The position also includes participation in an on-call rotation for occasional clinical needs.

Who You Are

Graduate of an approved school of practical nursing, currently licensed as a Licensed Practical Nurse in the State of Indiana

Two (2) years of experience in home health care or related health field (Preferred)

Car with required proof of insurance coverage and an Indiana Driver’s license

Knowledge of all types of home health equipment utilized by patients as well as forms and paperwork necessary for documentation

Requirements

Must be a graduate of an approved practical nursing school and currently licensed as an LPN in the State of Indiana. A valid Indiana driver's license, proof of insurance, and preferably two years of home health experience are required.
