In This Role You Will

The Registered Nurse Case Manager- Home Health

Perform comprehensive patient assessments and complete all required OASIS.

Develop, implement, and manage individualized Plans of Care in collaboration with physicians and the interdisciplinary team.

Provide skilled nursing care and serve as the primary case manager, coordinating services, monitoring patient progress, and communicating changes promptly.

Educate patients and caregivers on health conditions, safety, and self-care.

Ensure accurate, timely documentation and compliance with Medicare, state, and agency requirements.

Participate in case conferences and quality initiatives.

Provide on-call coverage, including evenings and weekends, as assigned.

Shift Details

This is a full-time, Monday through Friday position with daytime hours. While schedules offer flexibility to accommodate patient needs, RN Case Managers participate in a rotating on-call schedule, which includes evenings and weekends. Travel within our service area is required.

Who You Are

Current Registered Nurse (RN) license in the state of Indiana.

Minimum of 1–2 years of nursing experience (home health or acute care preferred).

Knowledge of OASIS documentation and Medicare home health regulations preferred.

Strong clinical assessment, critical thinking, and problem-solving skills.

Excellent organizational and communication skills.

Ability to work independently and manage a caseload effectively.

Valid driver’s license, reliable transportation, and proof of auto insurance.

Requirements
