About the role

Provide spiritual and emotional guidance to terminally ill patients, their families, and staff within an interdisciplinary team. Conduct worship services, rituals, and maintain accurate clinical documentation of patient care.

Full-time, 8:30am- 4:00pm with on call, weekend and holiday rotation.

Position requires travel in the community.

The majority of our patients prefer to receive hospice and palliative care at home to maintain their independence and live their lives to the best of their ability.

Symptoms are managed so that the patient can live a full life with the support of family and friends and the comforts of home.

Spiritual Care Counselors provide guidance for the spiritual and emotional needs of patients, families, and staff.

Participate in and conducts worship, including various ceremonies/rituals, during visits to new patients and follow up visits, as requested.

Works as part of an interdisciplinary team of care givers within HBI/CHPC network.

Essential Duties and Responsibilities for a Spiritual Care Counselor: Assess and provide for patient spiritual care needs, as well as those of primary care givers and other family members as appropriate in compliance with all organization, regulatory, and professional standards.

Ability to build rapport with patients and family members, utilizing effective communication skills with openness toward different lifestyles, cultures, beliefs, and values; and understand the value of being a non-judgmental, listening presence.

Collaborates with interdisciplinary groups to arrange and successfully implement patient driven plans of care (with particular responsibility for spiritual care needs.) Adheres to requirements of both CHPC and Hospice Buffalo policies and procedures that are both patient focused (clinical) and organizational focused (administrative).

Successful completion of 1 or more units of Clinical Pastoral Education (CPE) in coordination with management Board Certified Chaplain (or eligible for board certification) status preferred.

Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.

Experience Two (2) years of experience working in spiritual or pastoral care, specifically with terminally ill patients

Requirements

Requires a Master's degree in Divinity, Theology, or a related field, with a preference for Board Certification and Clinical Pastoral Education. Candidates must have at least two years of experience in spiritual care for terminally ill patients and a valid driver's license.
