Real Impact SummitWest Care is a local nonprofit home health agency seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled care to patients throughout Mesa County. This position offers flexible scheduling and the opportunity to build a caseload over time. You'll work independently in patient homes while receiving support from an experienced team of clinicians dedicated to delivering high-quality, patient-centered care designed to improve quality of life.
