This position offers the opportunity to build meaningful relationships with patients while helping them improve mobility, independence, and quality of life in the comfort of their own homes. Evaluate patients and develop individualized person-centered care plans Provide skilled physical therapy services in patient homes Improve patient mobility, strength, balance, and functional independence Educate patients and caregivers alike on treatment goals and home exercise programs Monitor patient progress and adjust treatment plans for individual goals Complete accurate and timely clinical documentation Collaborate with nurses, therapists, physicians, caregivers, and other members of the care team to ensure the best outcome
