PHYSICAL THERAPIST (PT) - Home Health SummitWest Care | Mesa County, CO Flexible Schedule | Full-Time or Part‑Time Competitive Hourly Pay (DOE) + Mileage Help Patients Thrive at Home SummitWest care is a local non-profit home health agency seeking a compassionate, skilled Physical Therapist to provide patient-centered care throughout Mesa County. This position offers the opportunity to build meaningful relationships with patients while helping them improve mobility, independence, and quality of life in the comfort of their own homes. You'll enjoy the autonomy of working in the field, while remaining connected and supported by a collaborative interdisciplinary team of clinicians. Our hourly pay structure allows you to focus on quality care and the best possible clinical outcomes for patients, rather than rushing from visit to visit.

Responsibilities

Evaluate patients and develop individualized person-centered care plans Provide skilled physical therapy services in patient homes Improve patient mobility, strength, balance, and functional independence Educate patients and caregivers alike on treatment goals and home exercise programs Monitor patient progress and adjust treatment plans for individual goals Complete accurate and timely clinical documentation Collaborate with nurses, therapists, physicians, caregivers, and other members of the care team to ensure the best outcome

Qualifications

Requirements

Must hold a current Colorado Physical Therapist license and be a graduate of an accredited Physical Therapy program. Requires a valid driver's license, reliable transportation, and strong clinical assessment skills.
