About the role

The Client Navigator coordinates with social workers and testers to provide healthcare navigation, insurance enrollment, and service linkage for participants. Responsibilities include conducting outreach, providing harm reduction education, and documenting client participation in the AWARDS system.

Job Summary

The HUB Navigator will coordinate with HUB Social Worker and HUB HIV Testers to provide client navigation to health care and other supportive services, service access advocacy, insurance enrollment, and confirmation of achievement of service linkages.

Primary Duties & Responsibilities include, but are not limited to

  • Ensure participants are engaged in services monthly
  • Remind participants of medical, MH appointments and assist with escorts when needed
  • Conduct on-on-one client level education with program participants
  • Ensure participants are reminded of group=level interventions
  • Conduct outreach activities to get new clients and those lost to care
  • Attend all trainings and webinars
  • Engage participant in harm reduction and risk reduction educational sessions, as well as provide pre and post counseling.
  • Ability to go out to required field sites and community events as scheduled and requested by management
  • Maintain and track all referrals (internal and external).
  • Maintain confidentiality at all times and appropriate record keeping
  • Tracks and documents client program participation
  • Enter data into AWARDS
  • Other duties as assigned.

Qualifications/Criteria

  • Experience conducting infectious disease testing (HIV/HCV).
  • At least 2 years of experience testing target population (women, homeless & people who inject drugs).
  • Familiar with Harm reduction practices.

Education

  • Minimum of a BA in Social Work or Community Health Education or related degree.
  • Or Associate degree with 2 year experience

Pay rate

$28.5714 per hour non exempt
