Provide comprehensive outpatient primary care services to adult and geriatric patients. Diagnose, treat, and manage acute and chronic medical conditions. Develop individualized care plans that promote preventive care, wellness, and disease management. Coordinate care with specialists, hospitals, and interdisciplinary care teams. Educate patients and caregivers on treatment plans, preventive health, and healthy lifestyle choices. Maintain accurate and timely medical documentation within the electronic medical record (EMR). Participate in quality improvement initiatives focused on value-based care and positive patient outcomes. Build strong, long-term relationships with patients and their families through compassionate, patient-centered care.
MD or DO from an accredited medical school. Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine. Active Florida medical license (or eligible for licensure). DEA license (or eligible). Current BLS certification. Experience with Medicare Advantage, managed care, value-based care, or geriatric populations is preferred but not required. New graduates are encouraged to apply. Bilingual (Spanish) skills are preferred for select locations.
