Ohio's Hospice is seeking a compassionate RN Team Nurse to join our interdisciplinary hospice team serving the Zanesville, Ohio area. In this role, you'll provide skilled nursing care to patients in their homes, assisted living communities, nursing facilities, and other care settings while supporting patients and families throughout their hospice journey.
As an RN Team Nurse, you'll collaborate with physicians, social workers, chaplains, hospice aides, and other healthcare professionals to deliver individualized, patient-centered care focused on comfort, dignity, and quality of life.
What you'll do
Provide skilled hospice nursing care through home and facility visits.
Complete comprehensive patient assessments and develop individualized care plans.
Manage pain and symptoms using evidence-based nursing practices.
Collaborate with the interdisciplinary team to coordinate high-quality patient care.
Educate patients and caregivers on medications, symptom management, disease progression, and available resources.
Accurately document patient care using the electronic medical record (EMR).
Advocate for patients while promoting comfort, dignity, and quality of life.
This is a field-based position requiring travel throughout the Zanesville service area.
