  • Practitioner, here’s a big-picture view of what you’ll do: Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms Complete F2F encounters for hospice patients as needed Develop and implement individualized care plans in collaboration with the interdisciplinary team Prescribe medications and coordinate care for patients with advanced or chronic illnesses Accurately document all encounters in the EMR Communicate effectively with patients, families, and other healthcare professionals

Requirements

Must hold an active Nurse Practitioner license in Wisconsin. While experience is preferred, the company is willing to train new Nurse Practitioners.
