We’re looking for full-time and part-time Personal Care Assistant's. We work 12-hour shifts, in the patient's home. Follows the Plan of Care to provide safe, competent care/services to the patient. Help the patient maintain good personal hygiene and assists in maintaining a healthy, safe environment. Plan and prepare nutritious meals. Assist the patient with ambulation as detailed by Patient Care Plan. Encourage the patient's independence but not exceed their capabilities outlined out in the Patient's Care Plan. Performs routine housekeeping tasks. Other duties as assigned.
